A Clinical Study on Acute Renal Failure

Abstract
A clinical observation was made on, 65 cases of acute renal failure that had been admitted to Han
Gang Sung Sim hospital during the 3 year period from January 1973 to December 1975. Following
results were obtained.
1) The morbidity to acute renal failure in adults was much higher in the young age group. The male to female ratio was approximately 1.6 : 1.
2) Acute renal failure occurs in a setting of sudden injury or illness usually associated¢¥¢¥¢¥ with shock or intensive renal vascular constriction. Common causes include Korean epidemic hemorrhagic fever, septicemia, acute glomerulonephritis, physical trauma and burn, etc.
3) In the majority of cases, clinical manifestations were characterized by transient oliguria, gastrointestinal symptoms and respiratory symptoms such as dyspnea.
4) The duration of oliguric phase varies (from several hours to a week but in most cases, the oliguric period was continued up to the second hospital day. At the time of admission, urine amount less than 400ml a day was seen in 18.5% of the cases. Proteinuria was noticed in 95.3% of the cases. The sodium concentration in expired group was 41.7mEq/l in average In 24 hour urine and was 24.7mEq/l in cured group.
5) At the time of hospitalization, blood chemistry revealed increased level of BUN and creatinine in all cases of the acute renla failure. Serum potassium was increased in 24.1% of the cases and serum sodium showed decreased level in 22.4% of the cases at the time of admission.
6) All patients disclosed mild anemia to normal hemoglobin level, significant thrombocytopenia and leukocytosis with leukemoid reaction were seen in most cases. Also, prolonged bleeiding time (in 25.8%) and coagulation time were seen.
7) On blood culture, microorganism were grown in 2 samples among 35 samples. In 18 cases of the 65 acute renall failure patients, elevated levels of transaminases were noticed.
$~ At the time of discharge, 61.5% of the cases showed improved clinical status, 10 7% of the cases were discharged in serious condition and 27.7% of cases were dead.
